5)Please give us a vet reference. If you do not have a vet reference, please explain why (moved, long time since last pet).

8)If you have children, have they been around dogs or puppies before?
9)How did you hear about us?
If it was a web site please let us know which site you found us on.
10)Do you own or rent your home?
11)If you rent your home have you checked to see if large breed pets are allowed?
12)If you rent please give your current landlord reference.
13)Your name.
14)Phone number and / or e-mail address.
15)How would you prefer to be contacted?
16)What state are you located in?
17)Please give the name of the puppy you are interested in.
18)Do you qualify for military, police or fire discount?
Past or current service.
19)Would you be interested in a Limited Registration Discount?
Limited Registration means the dog would be spayed or neutered before you ever received their papers and would never be able be able to register puppies to them.
20)A pet is a lifetime commitment. Are you prepared to welcome a dog into your home for up to
15 years?

Pre-Purchasing Questionnaire
Terms and Conditions
Please read these terms and let us know you agree before you send a deposit.
1)If your dog is being shipped - you MUST contact us within 30 minutes of receiving your puppy and let us know they have arrived safely.
2)If you can no longer care for your dog and you can not find an appropriate home for them, contact us. None of our dogs will EVER need to go to a shelter or pound!
3)Keep your dog healthy and up to date on vaccines.
4)Never chain your dog.
5)We reserve the right to refuse a puppy to anyone at anytime.
6)The deposit is nonrefundable.
7)Payment in full is due by the time the puppy is 7 weeks old. If paying with personal check, payment must be received by the time the puppy is 6 weeks old.
8)If you are paying for your puppy when you pick them up the only form of payment accepted is cash.
9)Preferred payment is Money Gram through Wal-Mart or MoneyGram.com.
10) Keep in touch - we expect a picture once or twice a year at least.
11) Please let us know if your dog passes away and why.
12)We love to receive pictures of your dog as it grows up. I publish several of them on our website or FaceBook page - if you do not want your pictures published, please let me know or else I will assume I can post them.
